If you are an adult and have fallen out of care with an adult CHD cardiologist, you are not alone.

There are over 165,000 adults in Canada living with CHD. It's estimated as high as 77% have been 'lost to care', meaning they are not being followed by a specialized adult congenital heart disease (ACHD) cardiologist.
